INDIVIDUAL PROVIDERS' INSURANCE FEES AND NETWORK BENEFITS ARE SUBJECT TO CHANGE. PLEASE CONTACT YOUR PROVIDER TO MAKE SURE THEY TAKE YOUR INSURANCE PRIOR TO SCHEDULING.
NOTE: Therapists who do not accept insurance may offer a "sliding" payment scale, and/or a form you can submit to your insuror for reimbursement. Check with the therapist.
